These are general health surveys used to conduct initial assessments to determine if there is a health risk. If it is determined that there is a health risk, we will then decide if there is a need for additional assessments. This is an integral part of the Social Health Maintenance Organization (SHMO) and was developed in compliance with CMS guidelines.

This process can be used independently or in conjunction with the Assessments and Evaluations tool. When integrated with the Assessment Object, collected data is automatically passed from the Health Risk Survey tool to the Assessments and Evaluations tool. Our Health Risk Surveys are Web enabled and can be operated over the Internet.
