Patterns of Care
Patterns of Care will focus on
evaluating the optimal care in regards to a
number of chronic conditions.
For example, if a patient has congestive heart
failure, Patterns will review the system data to
see if a patient is on an ACE Inhibitor, has a
cardiologist involved in care, evaluated for
routine physician visits, etc. Patterns will
then report on the gaps in the patient's care
patterns as identified.
